The Pennsylvania TCOM Collaborative supports TCOM implementations in Pennsylvania, and provides resources to help grow this data and quality movement for the benefit of Pennsylvania's children and families.
Providing coordinated, and organized care in a complex social services system is hard to do. Children and families can easily get lost in the system that is supposed to hold them when they are most in need. To overcome these barriers for Pennsylvania's children, families, and adults with mental illness, the PA TCOM Collaborative has formed to implement this internationally growing model for system value.
Transformational Collaborative Outcomes Management (AKA, "Total Clinical Outcomes Management" (Lyons, 2004)) is an open-source, research driven, outcomes management and human services management approach. Its basic tenants are for organizations and systems to use a uniform assessment at regular intervals, to guide treatment a new, as well as to quantitatively track the progress of care. TCOM is associated with the tools used for these assessment:
However, behind those tools is where the real force of TCOM lies. TCOM is built "bottom-up" as various implementations hone their own tool, and build up a data system that is locally relevant. To do this requires collaboration and communication across our many implementations so we can learn from each other, and achieve our ultimate goal of a unified and caring system of care for Pennsylvania's most vulnerable.
